Medical Social Services
Medical Social Workers evaluate social and emotional factors that affect the ill and disabled. They help patients and families manage the stress that many experience, and they connect them with available community resources. Medical social work is delivered in accordance with a physicians orders and as defined in a plan of care.

- Community Resource Planning and Referral
Home Health Aides
Home Health Aides assist patients in the completion of daily living activities and personal care. Many Home Health Aides are specially trained to deliver more complex services under the direction of a registered nurse.
Home Health Aides provide:
- Personal Care Assistance
- Mobility Assistance
Home Health Social Work and Aide services are available in conjunction with other Clinical care services. These services are available and can be arranged seven days a week, 8 a.m. - 4:30 p.m. For more information or assistance, contact the Intake Care Coordinator at 480-882-4222.
