Application Process

Memorandum of Understanding (MOU) / Training Affiliation Agreement (TAA) Scottsdale Healthcare has training agreements with the Air National Guard Bureau, Air Force Reserve Command, US Army Reserve, Western-Medical Area Readiness Support Group, Arizona Army National Guard as well as Luke and Davis Monthan Air Force bases. If your unit is unable to utilize existing agreements, you must generate a new Memorandum of Understanding (MOU) or Training Affiliation Agreement.

Mandatory Requirements for all participants

TB test: TB screening at SHC includes a 2-step testing process via Mantoux method with the two tests given 7-10 days apart. See explanation of criteria below.
  1. If you do not have evidence of a negative TB test within the last 12 months, the 2-test TB testing process, via Mantoux method, is required
  2. If you have been tested annually and can provide evidence of negative TB testing in the last 12 month, no TB test is required
  3. If you have a history of a positive reaction to a TB test, no TB test is required. You must have documentation of initial chest x-ray and must not have current signs and symptoms of disease as documented in a physician's note.
  • PLEASE NOTE: Duty uniform for Orientation Day and the lecture series is ACUs/DCUs
  • Military identification card
  • Individual shot record including MMR; Varicella and Hepatitis B
  • Original CPR card
  • Original licenses and/or certifications
  • Scrubs are required for clinical rotations. You may bring your own scrubs. If you do not have scrubs, Scottsdale Healthcare will supply scrubs while you are here
Optional Items

  • EMS response pouch (trauma scissors, penlight, etc.)
  • Stethoscopes—Scottsdale Healthcare does not provided stethoscopes. You are encouraged to bring your own

Mandatory Requirements for all doctors and physician assistants

All doctors and physician assistants need an Arizona license to practice medicine in Arizona.
For those who do not have an Arizona license, he/she must apply for a temporary (1-year) Arizona license through the following regulatory boards. You can find the appropriate temporary AZ license application (for training medical residency/fellowship) by following the links listed below. The Military Education Coordinator will help guide doctors and physicians to the correct contact for an application.

  • Medical Doctors/Physicians Assistants: The temporary medical license fee is $50. The temporary medical license application is available at the Arizona Medical Board and Arizona Regulatory Board of Physician Assistants PA
  • Doctor of Osteopathic Medicine: There is no temporary license or licensing fee required in the state of Arizona. According to ARIZONA REVISED STATUTES TITLE 32, CHAPTER 17 ARIZONA BOARD OF OSTEOPATHIC PHYSICIANS IN MEDICINE AND SURGERY, Article 2 32-1821 Persons and acts not affected by chapter.

    This chapter does not prevent:

    3. The practice by physician and surgeons discharging their duties while members of the armed forces of the United States or other federal agencies.

Additionally, the education coordinator will need the following information on each doctor or physician assistant:

  • Specialty
  • Verification of your Arizona state medical license (if appropriate)
  • Temporary AZ license (if required)
  • Verify completion of the following courses
    • Basic Life Support
    • Advanced Cardiac Life Support
    • Advanced Trauma Life Support
Other Considerations

Dress and Personal Appearance:
  • PLEASE NOTE: Duty uniform for the first 3 days of class is ACUs/DCUs.
  • Flight suits are not authorized while at Scottsdale Healthcare.
  • Scrubs are the duty uniform during clinical rotations. Scottsdale Healthcare will supply hospital scrubs for use during hospital shifts if you do not have scrubs. Please bring comfortable shoes, as you will be on your feet during most of your shifts. Tennis shoes may be worn with scrubs.
  • Dress as you would if you were on duty at a military hospital.
  • Nurses and technicians on EMS ride-along shifts must wear the ACUs/DCUs with boots.
  • Civilian casual is appropriate when not working. Urban wear is not acceptable.

    Meals
  • Meals are available at the Scottsdale Healthcare cafeterias as well as other area restaurants.
  • Your Scottsdale Healthcare Military Partnership badge will get you a 10% discount at the hospital cafeteria. Present your badge to the cashier to get your 10% discount.

    Transportation
  • Students from outside the local area should fly into Phoenix Sky Harbor Airport. Commercial ground transportation is available for travel between the airport and the hotels around Scottsdale Healthcare Osborn.
  • Shuttle service is probably the most efficient means of getting from the airport to your hotel. Super Shuttle is located in the baggage claim area at Phoenix Sky Harbor Airport.

    Force Protection
  • Scottsdale Healthcare is located in an urban setting. Hospital security personnel patrol the premises and appropriate security measures are deployed. Although these measures are important deterrents and protection, crime can and does occur in and around the Scottsdale Healthcare campus. Students are encouraged to exercise caution.

    Emergencies
  • Emergent injuries or illnesses will be seen at the Scottsdale Healthcare Osborn or the closest, appropriate, emergency room.
  • For personal or family emergencies, the program participant’s unit should initiate and process emergency leave requests. Our staff will assist in procuring emergency travel. Immediately notify the Scottsdale Healthcare administrative staff of any medical, personal or family emergency.

    Identification
  • Students will be provided Scottsdale Healthcare identification badges.
  • You must wear your badge at all times while participating in activities on the hospital campus.

    Military Bearing
  • During your rotation at Scottsdale Healthcare, remember you are a member of the US Armed Services and subject to regulations regarding appearance, fraternization and behavior.
  • Address fellow team members and faculty by proper military titles and civilian staff by “Mr./Ms./Dr.”
  • Exercise good judgment when choosing off duty activities.
