For New Staff Members
Quest Diagnostics will perform the Health Assessments for all New Hires and Newly Benefited Employees. Log into Quest's wellness web site below and complete the health questionnaire online and make an appointment for biometrics testing. You will have the choice of 8 Sonora Quest Facilities in the Valley to make a personal appointment, you must select a site on the available list to alert Quest to prepare your lab slip in anticipation of your visit.
In return for your participation, Scottsdale Healthcare will apply at $15 (per paycheck) Wellness Incentive towards your health plan premiums. Be sure to make your appointment as soon as possible. The sooner your testing is done, the sooner you will receive the incentive.
